So many people walk with arch pain as a result of too flat an arch or too high an arch. As we get older, the bones and ligaments that support the arch can reduce with gravity or become aggravated by overuse. Arch abnormalities can also cause heel or ball of the foot pain due to a shift in body weight. To make matters worse, the aging process causes a general thinning of the fat in the foot (especially in the ball of the foot) which is our body's natural cushion.

A bunion is a very misunderstood foot deformity. Most people see the prominence on the side of their great toe and assume that this represents a bump or lump on the bone of the great toe. The main purpose of the diagram is to show that the bunion is formed by the angle created by two bones.
Over the last decade, journals specializing in the medical care of the foot are very definite about the need to treat the abnormally flat-footed and pronated foot after age 3.
I often treat patients with hammertoes, toes that are bent, buckled or flexed at the main toe joint. A hammertoe procedure is performed by removing a small portion of bone. Hammertoes sometimes hurt from shoe contact or because of joint pain, indicating at times, arthritis. It's important to realize that a consultation concerning even conservative treatment is very important.
One of the most common complaints I hear in private practice concerns heel pain. What we in the medical field call heel spur syndrome usually involves a painful heel. Sometimes pain extends into the arch and the back of the heel in the achilles tendon insertion. Achilles tendon pain is at times accompanied with heel pain.

Sometimes pain extends into the arch and the back of the heel in the achilles tendon insertion. Achilles tendon pain is at times accompanied with heel pain. Although the symptoms vary, the source of the problem is the same: imbalance due to the pulling of a group of fibers (known as the plantar fascia) which inserts in the heel bone at the area where the heel spur is noted.
Many disorders can affect the joints in the toes, causing pain and preventing the foot from functioning as it should. A mallet toe occurs when the joint at the end of the toe cannot straighten. Excessive rubbing of the mallet toe against the top of the shoe can lead to pain and the development of a corn. The tip of the toe is often turned down against the shoe causing pressure and discomfort.
A neuroma, most commonly referred to as Morton's neuroma, is a swelling or enlargement of one of the small nerves of the foot that develops between two metatarsals. There are many reasons why a neuroma forms, some of which include the patient's foot type, shoe type and activity and, most significantly, from chronic injury to the nerve.
About 10 years ago I realized that when I made orthotics from a cast of peoples feet, they would wear them in their sneakers and laced shoes and then be without orthotics in all their other shoes. Orthotics control pronation and I realized it was therefore only being controlled some of the time and that to do right by my patients I had to provide orthotics that could be worn all the time. The answer: DRESSTHOTICS!
Patients who have a neuroma usually describe pain between the 2nd and 3rd toes, or 3rd and 4th toes. Pain usually starts at the base of the toes or in the web space and continues out toward the end of the toes. Most of the time pain is most pronounced during walking or activity, and is described as a sensation which alternates between aching, burning and numbness. Some patients feel the need to remove their shoes and rub the involved area for relief.
One of the most common complaints I hear in private practice concers pain in the heel. What we in the medical field call heel spur syndrome usually involves a painful heel. Heel pain, also known as plantar fasciitis, can be a result of chronic inflammation of the fascia itself or of a bursa (small, fluid-filled sac) that may be present. In addition, as we age, the fat pad of the heel becomes thinner and degenerates, which can lead to inadequate padding on the heel and chronic pain in this area. Also, occasionally the small nerves that run along the plantar fascia on their way to the forefoot can become irritated and painful.
n simple terms, pronation is the flattening out of the arch when the foot strikes the ground. Normally, the foot will pronate to absorb shock when the heel hits the ground, and to assist in balance during mid-stance. The ankle will 'tip' towards the inside.
Very often non-prescription (non-casted) orthotics have enough inherent control to adequately control pronation. Thus, my discovery of dressthotics, which are wafer-thin and can be worn in all types of shoes for the vast majority of patients. The control inherent in dressthotics is adequate and they don't require prescription orthotic devices. Sometimes the comfort that patients get just in dressthotics steer them into realizing that orthotics made from a cast of their feet is the appropriate direction for treatment.
Supination is the opposite motion of pronation. A foot is in supination when the ankle appears to be 'tipped' to the outside so you are standing on the outside border of the foot. Supination allows the foot to be a more stable, rigid structure for when we push off on our next step.
A bunionette is similar to a bunion, but is found on the outside of the foot. It is sometimes referred to as a Tailor's bunion due to the sitting position of tailors while working, cross legged with the outside edge of their feet rubbing on the ground. This produces a pressure area and callus on the outer edge of their feet at the area of the fifth toe joint.
Dr. Shapiro is known for her dedication (and the number of hours she spends in the office). She has been known to remark, "I have been truly blessed because I love what I do."
